The role of leptin in human obesity and disease: a review of current evidence.

چکیده
PURPOSE To review recent advances in the pathophysiology and potential clinical applications of leptin, an adipose tissue-derived hormone. DATA SOURCES A MEDLINE search of the literature on leptin and the bibliographies of relevant papers. STUDY SELECTION All 1320 publications on leptin. DATA EXTRACTION All identified articles were reviewed. Cited publications were selected on the basis of study quality and relevance to human obesity and disease. DATA SYNTHESIS Leptin is a 16-kilodalton adipocyte-derived hormone that circulates in the serum in the free and bound form. Serum levels of leptin reflect the amount of energy stored in adipose tissue. Short-term energy imbalance as well as serum levels of several cytokines and hormones influence circulating leptin levels. Leptin acts by binding to specific receptors in the hypothalamus to alter the expression of several neuropeptides that regulate neuroendocrine function and energy intake and expenditure. Thus, leptin plays an important role in the pathogenesis of obesity and eating disorders and is thought to mediate the neuroendocrine response to food deprivation. Phase I and II trials recently showed that leptin administration to humans is safe, and ongoing phase III trials are assessing the efficacy of leptin as a treatment for obesity and related disorders. Availability of leptin or smaller and more soluble leptin analogues for clinical studies in humans is expected to significantly advance understanding of the mechanisms underlying energy homeostasis in humans. CONCLUSIONS Leptin is significantly broadening our understanding of the mechanisms underlying neuroendocrine function, body weight, and energy homeostasis. Elucidation of these mechanisms is expected to result in the development of novel therapeutic approaches for obesity and eating disorders.

Genes Predisposing to Monogenic, Polygenic, and Syndromic Obesity: A Review of Current Trends and Prospects for Standard Obesity Genetic Testing
Objective: The burden of obesity is currently enormous, necessitating a novel strategy to complement the existing ones. Accordingly, genetic predisposition is suspected in many cases of the disease, which can potentially be used as therapeutic targets. However, there are differing viewpoints on the suspect genes, prompting the current review to articulate the genes and their mechanisms. Eight (...
متن کامللپتین: هورمون نویافته بافت چربی و اثرات آن بر چاقی
Leptin is a 16-kD protein which is secreted from white adipocytes and, its discovery has generated enormous interest in the regulation of energy balance. Leptin has been implicated in the regulation of food intake, energy expenditure, and whole-body energy balance in animals and human. Plasma leptin levels correlate with fat storages and respond to changes in energy balance. It was initially pr...
متن کاملFunctions of Obesity-related Hormones in Breast Cancer: A Review
ABSTRACT Obesity is a health problem defined as abnormal or excessive fat accumulation in adipose tissue. Adipokines such as adiponectin, visfatin and resistin are bioactive polypeptides with pro- and anti-inflammatory properties that are secreted by the adipose tissue that are involved in the pathogenesis and prognosis of diseases ...
متن کاملP 121: Neuroinflammation and Diabetes
Obesity is a chronic disease that shows the most serious global health problems .It relates to the body fat .Obesity is caused by an imbalance between energy intake and expenditure, this balance is regulated by genetic and environmental interactions. Obesity, which is a risk factor for chronic diseases, has become epidemic in the developed countries. Resistance to Leptin hormone is one of the m...
متن کاملP99: The Role of C- Reactive Protein in Obesity and Neuropathic Diseases
Acute phase C-reactive protein (CRP), raised in obesity and inflammation, is a main binding protein for leptin. It is assumed that CRP contributes to leptin confrontation by preventing leptin from crossing the blood-brain barrier (BBB). Here we defined how CRP cooperates with the BBB and whether it deters leptin from attainment CNS targets. CRP was constant in blood, but did not permeate the BB...
متن کاملRegulation of Appetite: Role of Serotonin and Hypothalamus
Serotonin (5-HT), a mono-aminergic neurotransmitter is biochemically derived from tryptophan and is mainly found in gastrointestinal tract, platelets and central nervous system of animals. Serotonin (5-HT) in coordination with hypothalamus plays an important role in the CNS control of appetite, eating behavior, and energy balance and body weight. It has a special role in control of carbohydrate...
